Our Providers

Lindsey Smith
MSN, APRN, FNP-C, CDP
Lindsey is originally from Rock Hill, South Carolina. She earned a Bachelor of Arts in Spanish Language and Literature from North Carolina State University, a Bachelor of Science in Nursing from Queens University, and a Master of Science in Nursing from Clemson University. She is also a Certified Dementia Practitioner.
Lindsey moved to Greenville in 2018 and began her career in the Upstate, working in home health with the geriatric population in assisted living facilities, where she developed a deep passion for supporting one of our most vulnerable groups. Recognizing the significant gap in mental health care for older adults—who often face complex emotions and challenging life transitions—she is dedicated to helping them navigate these changes with empathy and support.
Lindsey takes a collaborative approach to care, working closely with patients, families, facility staff, and evidence-based practices to create care plans that serve everyone involved. In her free time, she enjoys spending time with her family and friends, going to the beach, and walking her dogs, Bob Barker and Sadie.

Caroline West
MSN, APRN, AGNP-C, CDP
Caroline West is originally from Savannah, Georgia. She earned her Nursing degree from the University of South Carolina and later completed her Master of Science in Nursing at South University, graduating in 2003 with a focus in Adult-Gerontology.
Caroline has a deep appreciation for hearing each patient’s life story and holds a strong awareness of how often mental health concerns in the elderly are overlooked or mislabeled. She enjoys the challenge of balancing holistic and clinical interventions to provide comfort, reassurance, and clarity for both patients and their families. Caroline believes that warm conversation and active listening are essential components of effective care, and she prioritizes meaningful dialogue and education as part of her approach.
In her free time, she enjoys movies, reading, thrifting, traveling, and exploring new restaurants.

Hannah David
MSPAS, PA-C, CDP
Hannah was born and raised in Northeast Georgia. She completed her Bachelor of Science in Interdisciplinary Studies with a focus in Biomedical Ethics at Young Harris College before earning her Master of Physician Assistant Studies (MSPAS) from Brenau University in Gainesville, Georgia. She holds certifications in Basic Life Support, MOCA, Clozaril REMS, and Medication-Assisted Treatment.
Hannah moved to the Seneca–Clemson area two years ago and is passionate about caring for the geriatric population, recognizing the unique challenges this stage of life brings—such as loss of independence, loneliness, and complex chronic conditions that often require thoughtful support. She values iTrust Cares’ specialized, high-touch approach, including in-person follow-up visits that provide deeper insight into mental health concerns, especially for patients navigating comorbidities.
With every client, Hannah strives to understand the full picture of their life to guide accurate diagnosis and treatment, working closely with family, caregivers, and loved ones whose perspectives are essential to building effective care plans. In her free time, she enjoys reading across genres, spending meaningful time with her family, and traveling both locally and around the world.

Renee Bridges
MSN, APRN, FNP-C, CDP
Renee is originally from a small town in Illinois but moved to the Upstate of South Carolina at the age of six, where she has lived for most of her life. She also spent four years in Norfolk, Virginia, while her husband was in the military, before relocating back to South Carolina in 1997. Nursing is Renee’s second career; she earned her BSN from the University of South Carolina Upstate in 2009 and later completed her Master of Science in Nursing at Walden University, graduating in February 2018. She holds certifications as an APRN, a Certified Dementia Provider (CDP), and a Certified Family Nurse Practitioner (FNP-C).
Since becoming a nurse practitioner, Renee has devoted her career to caring for the geriatric population, finding the work deeply meaningful and rewarding. She believes small interventions—whether adjustments to daily routines or simply taking the time to listen—can greatly enhance a patient’s quality of life, and she values being able to witness those improvements.
Renee’s approach is to care for each person as she would a loved one, prioritizing meaningful conversation and trust-building over a purely clinical interaction. In her free time, Renee enjoys spending time with her family and her puppy, always keeps a book nearby for a few stolen minutes of reading, and loves doing crafts when she has the time.

Melissa Danner
MSN, APRN, AGNP-BC, CDP
Misty was born in Sumter, SC, on Shaw Air Force Base, and has lived in the Upstate of South Carolina for more than 20 years. She earned her undergraduate degree in Biology from USC Upstate and completed both her nursing degree and Master’s degree at Clemson University. She holds certifications in Advanced Life Support and as a Dementia Specialist.
Misty has always had a deep passion for senior mental health, recognizing how often this population is overlooked; she believes that aging adults should be treasured, supported, and reminded that they are valued members of their community. Her approach to care focuses on helping seniors remain proactive in their own well-being while offering guidance, encouragement, and medication management support when needed.
In her free time, she enjoys spending quality time with her family and loves doing anything outdoors, including hiking, biking, gardening, or swimming.

Lily Rigmaiden
PA
Lily is originally from Denver, Colorado, where she grew up hiking, camping, and spending time outdoors. She earned her Bachelor’s degree in Criminal Justice and Psychology from the University of Northern Colorado and went on to complete her Master of Physician Assistant Studies at Drexel University. She is also certified through the NEI Master Psychopharmacology Program.
Lily has a special passion for working with the geriatric population and deeply values their stories, perspectives, and experiences; she is committed to providing safe, effective, and collaborative care alongside families, staff, and support teams to help patients achieve the highest quality of life.
Lily moved to the Upstate with her husband and son in June 2024, and in her free time, she enjoys enriching her toddler’s life through activities and small trips, spending quality time with her dog and cat, and pursuing hobbies like reading, kayaking, camping, and traveling.
